OUR POLICIES

During each appointment, a child needs to be accompanied by a parent or legal guardian. Please have your picture ID or copy of legal guardianship available for each appointment. If you plan on having someone else bring your child to his or her appointment, you will need to complete a notarized medical power of attorney prior to the appointment. For your convenience, we have a notary on staff.

Our office makes every attempt to remain on schedule throughout the day. We know that your time is valuable and we will do our best to keep you and your child from having to wait. As a courtesy, our office will attempt to contact you for confirmation 1-2 days before your appointment. However, we do ask that parents and legal guardians assume responsibility for your child’s appointment time.

Broken appointments or short term cancellations (within 48 hours) without proper notification can be costly and unfair to other patients who need appointments. Repeated broken appointments of any kind may warrant dismissal from the practice. If you are running late, please notify us. We usually try to work any late arrivals back into our schedule when time allows, or reschedule for another day at your convenience.

It is necessary that we work by appointments. Unfortunately, emergencies do occur which cause delays in our schedule. However, we will try our best to honor your appointment time. In regard to emergency treatment, we are available 24 hours a day simply by calling our office.

Credit-CardsFees for dental services are due on the date of treatment unless prior arrangements have been made with our financial coordinator. Our office accepts cash, checks, MasterCard, Visa, Discover and American Express. We offer a variety of options on extensive treatment plans that may better fit your budget.

Our office, as a courtesy, will file dental insurance benefits for you, however, you will be responsible for any deductibles, co-payments, or balances not covered by insurance. Our office will attempt to call ahead and inform you of the estimated costs of treatment, to avoid any surprises. All account balances which have not been paid within a 30 day period become due by the parent/guardian responsible.

Insurance payment is determined by “UCR” fees (usual, customary, and reasonable fees). These fees are not always the same as our fees. Some insurance companies may pay less, some pay more. Whether your insurance pays 100%, 80%, or 50% of a procedure, they are determining payment based on their fee schedule, not the actual fee our office has charged for the service.

Please keep us informed of any changes or updates in your insurance information so that we can continue to assist you with fast and accurate insurance filing.

GEORGIA MEDICAID & GEORGIA PEACHCARE

Our office accepts Georgia Medicaid and Peach Care for Kids. Please have the current month’s card showing eligibility prior to your visit. If for some reason you do not receive your card before your visit, please call our office so we can verify coverage over the telephone.
