During each appointment, a child needs to be accompanied by a parent or legal guardian. Please have your picture ID or copy of legal guardianship available for each appointment. If you plan on having someone else bring your child to his or her appointment, you will need to complete a notarized medical power of attorney prior to the appointment. For your convenience, we have a notary on staff.
Our office makes every attempt to remain on schedule throughout the day. We know that your time is valuable and we will do our best to keep you and your child from having to wait. As a courtesy, our office will attempt to contact you for confirmation 1-2 days before your appointment. However, we do ask that parents and legal guardians assume responsibility for your child’s appointment time.
Broken appointments or short term cancellations (within 48 hours) without proper notification can be costly and unfair to other patients who need appointments. Repeated broken appointments of any kind may warrant dismissal from the practice. If you are running late, please notify us. We usually try to work any late arrivals back into our schedule when time allows, or reschedule for another day at your convenience.
It is necessary that we work by appointments. Unfortunately, emergencies do occur which cause delays in our schedule. However, we will try our best to honor your appointment time. In regard to emergency treatment, we are available 24 hours a day simply by calling our office.
